Using the Sick Child and Emergency/Back-Up Care Program

The registration process consists of two steps:

  1. Complete and submit your UA Student or Employee Registration Form to Life & Work Connections.
  2. Complete CHOICE OPTIONS, INC.'s (CO) Vendor Registration Form and return it directly to CO in Tucson.

Registrants must complete both steps to access program services and subsidized fees. Student rates are valid for the 2011/12 SCP/EBCP service year period of July 1-June 30. All caregiver transportation costs within the greater Tucson and Phoenix areas are absorbed by CO. Please contact CO at (520) 638-6538 to verify if additional transportation costs may be applicable for your child care location.
