Enrollment Verification Form

Completed by your provider, you must submit an Enrollment Verification Form with your application. This verifies your child’s enrollment in childcare.

Answering your questions about Childcare Choice – Student

The Childcare Choice – Student program offers up to $2,000 childcare subsidy awards per fiscal year per household. The childcare must occur in Arizona, and it must be coursework related (i.e., necessary to allow you to attend class or complete coursework). To participate, you must be a current undergraduate or graduate/professional student at the University of Arizona.

Yes. All eligible students must submit a new application each fiscal year.

Yes. Students approved during the fall semester must email a new Enrollment Verification Form to ccstudent@arizona.edu in January to receive the spring semester award.

No. A child’s birthdate and Enrollment Verification Form are both needed to apply for this program.

Childcare Choice awards are given per household. Awards do not vary based on the number of children within the household.
