Flu shot clinics

Eligibility Criteria:

  • All Benefit Options Members (enrolled in an ADOA medical plan) age 4 and older including: employees, retirees, spouse, and dependents.
  • State Employees Eligible for Full Benefits (those who were offered ADOA medical benefits, even if they declined coverage)
  • FOR THOSE NOT BENEFITS ELIGIBLE THE CHARGE IS $40.00


To receive a Flu Shot you will need:

  • Print and fill out the Flu Shot Consent Form
  • Bring the completed form, your insurance card, and employee ID number  

IMPORTANT!!

Employee ID (EIN) required. This is NOT your Cat Card number. Your EIN can be found on your time sheet.
